What You'll Do Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings. Complete nursing assessments, symptom management, medication education, and comfort-focused interventions. Support patients and families through education, advocacy, and compassionate communication. Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team. Complete accurate, timely EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care exposure. Excellent assessment, communication, and time-management skills. Ability to work independently in a field-based role. Reliable transportation and a valid driver's license.
