About the Role As a Case Coordinator, you will provide individualized skills training and support to youth, families, and adults in a variety of settings—including homes, schools, and the community. You’ll play a key role in helping clients build daily living skills, improve social functioning, and increase independence, all while fostering meaningful relationships and measurable progress. What You’ll Do Develop and implement personalized training plans based on each client’s unique goals and needs Provide engaging one-on-one and group sessions focused on daily living skills, social skills, and parenting education Apply evidence-based practices and creative approaches to support learning and growth Offer ongoing mentorship and encouragement to clients in a supportive, positive environment Help clients apply new skills in real-world situations to build confidence and independence Collaborate closely with families, caregivers, and service providers to ensure holistic care Participate in team meetings and case conferences to monitor progress and adjust plans Maintain accurate documentation, progress notes, and reports Connect clients with community resources and support their participation in community activities Promote social engagement and community integration Stay current with best practices through ongoing training and professional development If you’re ready to make a lasting impact and help others build brighter futures, we’d love to hear from you!
