About the role
Position Purpose: The Pharmacist is responsible for educating, assisting, and interacting with patients in an outpatient clinic setting with regards to health promotion and wellness. Key Functions and Responsibilities Works collaboratively with multi-disciplinary teams to optimize drug therapy. Provide direct patient care, including treatments assessment and medication management activities. Clinically evaluate all patients in a specified area daily. Ensure documentation is placed in the medical record regarding pharmacy recommendations, plans, intervention and medication reconciliation in medical records. Review patient’s drug regimen for an appropriate indication, dosing, allergy verification, potential drug interactions, lab monitoring, duplication of therapy, and medical documentation. Provide accurate and timely drug information to physicians, nurses and other health professionals. Respond to level 1 trauma, code blue massive blood transfusion emergencies. Provide direct patient care and medication therapy monitoring, including antibiotic stewardship, pharmacokinetic drug dosing, renal and hepatic dosing and patient education. Develop, implement, and monitor clinical protocols. Provide consultative services and clinical expertise to evaluate and improve medication usage and therapeutic outcomes. Process orders and prescriptions to provide medication and pharmaceutical supplies ordered by physicians and authorized medical practitioners. Facilitate discharge planning and smooth transition to alternate sites for care from a pharmacy perspective. Provide cost-effective management of drug utilization within the clinic. Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects. Provide drug information to patients and their families. Demonstrate professional communication with patients, family members, professional and support personnel involved in the delivery of patient care.
