About the role
ARC Outreach Practitioner II Access and Recovery Center (ARC) Outreach is a wraparound service integrating ARMHS, and/or Outreach services with clients. This position is responsible for providing client engagement and direct client services and interventions through ARC Outreach. Additionally, this position provides access coordination through a seamless progression for individuals entering services by guiding them through the intake process, supporting consistent engagement, and rapidly addressing barriers to access. This role is focused on acting as a bridge between initial contact and long-term care coordination. It also provides basic needs support and referrals in community settings, Community Support Programs/drop-in spaces, and places not meant for habitation. This is executed by exercising a person-centered approach to client support and services, performing assessments, providing education, promoting healthy lifestyles, facilitating individual and/or group meetings, clear and timely documentation and delivering individualized plan of care consistent with client goals and objectives. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions Main Job Duties: Client Engagement & Person-Centered Support Build and maintain therapeutic, person-centered relationships that promote client recovery and goal attainment. Maintain regular, active contact with clients to support service utilization and evaluate progress. Provide skill-building, teaching, and coaching aligned with client treatment plans and goals. Service Delivery & Intervention- Facilitate and conduct individual services addressing mental health, behavioral, and wellness needs; provide group interventions as applicable. Deliver crisis intervention and de-escalation strategies to ensure client and program safety. Guide clients through processes related to accessing services, resources, and benefits. Service Coordination, Collaboration and Transition Support Manage and track internal and external referrals from the evaluation process. Facilitate and coordinate connections to services, including “hand-off” to long-term care coordination teams. Maintain communication with individuals and service providers to ensure successful initiation of services. Documentation & Record Management Document all client interactions, services, and interventions in the Electronic Health Record (EHR) or designated systems according to program standards. Maintain accurate, timely case notes and ensure all activities are properly recorded and compliant.
