What We Offer: • Mileage reimbursement • Competitive salary • Comprehensive benefits package, including: ◦ EAP ◦ 401(K) with 4% match • Ongoing professional development and career advancement opportunities • A collaborative, mission-driven team committed to delivering exceptional patient care Location: South San Jose Schedule: Monday - Friday with on call weekends rotation As a Home Health ST, you will: • Evaluate patients’ speech, language, communication, and swallowing-related needs, including assessing the home environment and identifying appropriate equipment. • Develop, implement, and update individualized speech therapy treatment plans and Plans of Care (POC) in collaboration with the physician and care team. • Provide skilled speech therapy services using evidence-based therapeutic techniques and specialized equipment to improve functional outcomes and quality of life. • Monitor patient progress and communicate changes in condition, treatment response, and functional status with the physician, case manager, and interdisciplinary care team. • Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements. • Educate patients, caregivers, and other healthcare personnel on appropriate therapeutic strategies, communication techniques, and ongoing care needs. Required Qualifications • Current state licensure as a Speech Language Pathologist • Master’s or Doctoral degree in Speech Language Pathology • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting • Current CPR/BLS certification • Valid driver’s license and insured personal vehicle (per Lorian requirements) This position is based on a pay per visit compensation model. Pay per visit ranges from $120-$125 visit commensurate with experience and depending on type of visit.
