JOB SUMMARY In October 2020, the Philadelphia Department of Public Health (PDPH), Division of Maternal, Children and Family Health (MCFH) launched Philly Families CAN. Philly Families CAN is a referral service for families that live anywhere in Philadelphia to learn more about and get connected to programs that best match their needs and interests, at no cost to the family. Philly Families CAN supports families with children aged 0-17 by connecting them to free, voluntary programs that support caregivers and their families. Philly Families CAN consists of both a phone number and a website ( PhillyFamiliesCAN.com ) through which families and referring providers can access services. Family-choice is a core value of Philly Families CAN; all services are voluntary. The goal of this program is to advance health equity and health justice, so that every family and child in Philadelphia, especially in communities impacted by systemic oppression, has access to the resources and support to thrive. The Philly Families CAN Engagement Coordinator will be joining the Philadelphia Department of Public Health (PDPH), Division of Reproductive Adolescent, and Child Health (ReACH) and will report to the Philly Families CAN Manager. This individual will be responsible for engaging and maintaining strong relationships between the program and a community of referring providers (such as teachers, social workers, and doctors). The Engagement Coordinator will also coordinate and participate in community outreach events to spread the word about Philly Families CAN. JOB SPECIFICATIONS Responsibilities/Duties Referring Provider Engagement Form and maintain strong working relationships with a network of over 500 community referring providers at schools, hospitals, healthcare centers, daycares, and other community-based organizations serving families and pregnant people. Create and lead presentations and webinars to introduce potential referring providers to Philly Families CAN. Facilitate workshops and host meetings with existing referring providers to provide updates and trainings for referring providers. Develop and maintain system to document and track referring provider engagement. Gauge and document opportunities for referral-process improvement. Collaborate with Intake Coordinators, providers, and MCFH Systems Administrator to update referral processes and referring-provider training. Review data with MCFH Lead Epidemiologist to identify patterns and gaps in referral sources. Concrete Goods Fund Support the Concrete Goods Specialist and Applications Administrator in public messaging and referral improvements to the Concrete Goods Fund. Create Concrete Goods trainings about how providers can make referrals to the Concrete Goods Fund and the complete application process. Engage with providers on a regular basis about any process improvements, changes, or updates about the direction of the Concrete Goods Fund. Outreach Coordination Develop and maintain an outreach schedule for community outreach events and referring provider engagement and training. Search out relevant community events for Philly Families CAN to share materials and information. Coordinate attendance and communication with host organizations, maintain relationships with organizations for upcoming events and opportunities. Collaborate with MCFH Communications Manager on messaging for providers to align with Philly Families CAN awareness campaign. Coordinate dissemination of program materials with MCFH Communications Manager and Intake Coordinators. Administrative Tasks Check voicemails and respond to inquiries to the General Question voicemail box. Attend Philly Families CAN team check-ins to ensure a thorough and up-to-date understanding of the services to which Philly Families CAN refers. Attend Centralized Intake Steering Committee meetings and provide updates, such as champion referring providers and referral process updates. Manage swag inventory and place orders as necessary Steering Committee administration Outreach to prospective programs. Manage and track onboarding and approval process for new partner programs. Assist with annual stipend and MOU management. Other duties as assigned.
