CLINICAL AMBULATORY PHARMACIST

JOB SUMMARY Provides clinical pharmacy services, including drug information, pharmacokinetics, patient education, and participation in multidisciplinary patient care groups within the Congestive Heart Failure Clinic and the Cardiology Office. Goals with future plans on expanding services to other clinics. Ensures the accurate and appropriate use of pharmaceuticals for patients of all ages in the hospital within the standards of professional practice and in accordance with the policies and procedures of the Pharmacy.
