About the role
POSITION SUMMARY Under the direction of the Parent Coaching Program Manager, the Family Advocate provides comprehensive family strengthening services to children and families participating in the Early Childhood Wrap Services Program. The Family Advocate partners with families to promote healthy child development, strengthen protective factors, and connect families with community resources through home visitation, developmental screenings, parent education, case management, and family goal planning. Responsibilities include outreach, recruitment, enrollment, ongoing case management, facilitation of parent groups, and maintaining a caseload of approximately 20 families while ensuring compliance with contract requirements and CAPC standards. Maintain regular and reliable attendance. Perform other agency and program-related duties as assigned. PROGRAM SPECIFIC DUTIES Family Engagement Conduct outreach, recruitment, intake, and enrollment activities. Develop trusting, family-centered relationships. Maintain an assigned caseload of approximately 20 families. Conduct weekly home visits using approved evidence-based curriculum (Parents as Teachers) while maintaining program fidelity. Assist families in developing and achieving individualized family goals. Empower parents to advocate for themselves and their children. Child Development Conduct Ages & Stages Questionnaires (ASQ) and other required developmental and behavioral screenings. Educate parents regarding child development, attachment, and positive parenting practices. Promote safe, healthy, and nurturing home environments. Case Management Complete assessments including the Protective Factors Assessment. Maintain accurate case files and documentation. Maintain required tracking databases and reports. Provide referrals and follow-up with community resources. Collaboration Facilitate monthly parent groups or family connection activities. Participate in staff meetings, trainings, case conferences, and required contract meetings. Represent CAPC at community events and collaborate with partner agencies.
