Case Manager, Community Treatment

Work Schedule: Monday - Friday 8:30am - 4:30pm with on call hours. Chesapeake Connections helps people achieve success in their community through the right combination of mental health services and support programs. Our team-based approach to care provides creative solutions for client’s needs, providing the best chance at success in their treatment or recovery. This direct care position will be responsible for coordinating and providing comprehensive care and support for assigned clients.
