What You'll Do Provide routine and PRN hospice visits in patients' homes, assisted living, memory care, and skilled nursing communities. Perform comprehensive nursing assessments, symptom management, wound care, and medication education. Support patients and families through every stage of the hospice journey. Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team. Complete accurate and timely EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience preferred, or strong experience in oncology, palliative care, home health, ICU, or end-of-life care. Strong assessment, communication, and critical thinking skills. Ability to work independently in a field-based role. Reliable transportation and a valid driver's license.
