Physical Therapist (PT) Chandler Creek Post Acute – Greer, SC $45–$50/hour | Full-Time | Day Shift About Us Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We are seeking a dedicated Physical Therapist (PT) to join our full-time rehab team. What We Offer Competitive pay: $45–$50/hour Day shift schedule for work-life balance Comprehensive benefits package: Medical, dental, vision, and life insurance PTO + 401(k) with company match PRN opportunities within the largest network in South Carolina Supportive, collaborative rehab environment Position Summary As a Physical Therapist (PT) , you will evaluate, develop, and implement individualized treatment plans to help residents improve mobility, reduce pain, and achieve their highest level of independence. Key Responsibilities Conduct patient evaluations and develop customized treatment plans Provide therapeutic interventions to improve strength, mobility, and function Monitor patient progress and adjust treatment plans as needed Educate residents and families on therapy techniques and goals Maintain accurate and timely clinical documentation Collaborate with interdisciplinary teams to ensure optimal patient outcomes Qualifications Bachelor’s degree required ; advanced degree preferred Active, unencumbered Physical Therapist license in South Carolina Experience in a long-term care or rehab setting preferred Familiarity with PointClickCare and Casamba preferred Strong communication and clinical decision-making skills Passion for patient-centered care Why Join Chandler Creek Post Acute?
