RN - Home Health- Admission Liaison- Infusion

As a Registered Nurse-Admission Liason for Infusion, you would be tasked to Assess patients individually for service needs once patient choice has been elicited from appropriate hospital staff and/or referral is made to MH from other referral sources. The Admission Liaison is accountable for either home care or infusion needs assessments, as assigned.
